Advanced Cryobiopsy for Accurate Lung Diagnosis

Cryobiopsy is an advanced minimally invasive procedure used to obtain relatively larger and better-preserved tissue samples from the lungs compared with conventional small forceps biopsies. It uses a specialized cryoprobe to temporarily freeze lung tissue, allowing the physician to obtain a tissue sample for detailed laboratory and pathological examination.

What Is a Lung Cryobiopsy?

A Transbronchial Lung Cryobiopsy (TBLC) is performed through a bronchoscope. A small cryoprobe is guided through the airways to a carefully selected area of the lung. The probe briefly freezes the surrounding tissue, allowing a larger tissue sample to be obtained.

The sample is then examined by a pathologist. When combined with clinical information, HRCT findings and other investigations, cryobiopsy can contribute to a more confident diagnosis in selected patients with undiagnosed interstitial lung diseases (ILDs).

When Is Cryobiopsy Recommended?

Cryobiopsy may be considered when non-invasive investigations and multidisciplinary assessment do not provide a sufficiently confident diagnosis and tissue sampling is clinically indicated.

It may be used particularly in the evaluation of selected:

  • Interstitial Lung Diseases (ILDs)
  • Diffuse parenchymal lung diseases
  • Unexplained lung fibrosis
  • Inflammatory lung disorders
  • Other complex diffuse lung conditions

Not every patient with ILD requires a biopsy. The decision should be based on clinical findings, HRCT patterns, pulmonary function, overall health and multidisciplinary discussion.

How Is the Procedure Performed?

Cryobiopsy is generally performed using bronchoscopy under appropriate sedation or anaesthesia. The patient is carefully monitored throughout the procedure.

The pulmonologist uses imaging and bronchoscopic findings to select an appropriate area for sampling. The cryoprobe is introduced through the bronchoscope and briefly activated to freeze the target tissue. The frozen specimen is then retrieved and sent for pathological examination.

The exact technique, airway management and number of samples depend on the patient's condition and the expertise of the treating centre.

Benefits of Cryobiopsy

For appropriately selected patients, transbronchial cryobiopsy can provide useful tissue samples while avoiding surgical lung biopsy in some circumstances. The ERS guideline suggests TBLC as a tissue-sampling option for selected patients with undiagnosed ILD when histopathological information is required.

Potential benefits include:

  • Larger tissue samples than conventional forceps biopsy
  • Improved preservation of lung architecture
  • Minimally invasive bronchoscopic approach
  • Useful information for selected ILD cases
  • May reduce the need for surgical biopsy in appropriate patients

Safety and Possible Risks

Although cryobiopsy is minimally invasive, it is not risk-free. Possible complications include bleeding, pneumothorax and, less commonly, worsening of the underlying lung disease. Published studies show that complication rates can vary considerably depending on patient selection, technique and centre experience.

For this reason, cryobiopsy should be performed by appropriately trained specialists in centres equipped to manage potential complications.
