Advanced Thoracoscopy for Pleural Disease Diagnosis and Treatment

Thoracoscopy, also known as pleuroscopy in the medical setting, is a minimally invasive procedure that allows a pulmonologist to directly examine the pleural space—the area between the lungs and chest wall. It can be used to investigate unexplained pleural conditions, obtain tissue samples and, in selected patients, treat recurrent pleural fluid.

What Is Thoracoscopy?

During medical thoracoscopy, a small thoracoscope is introduced through a small opening in the chest wall. This allows the doctor to directly visualize the pleura and take targeted biopsies when necessary.

Thoracoscopy may provide more direct information about pleural abnormalities than imaging alone. Tissue samples can be sent for laboratory and pathological examination to help determine the underlying cause.

When Is Thoracoscopy Recommended?

Thoracoscopy may be considered when other investigations, such as chest imaging and pleural fluid analysis, have not established a diagnosis.

Common indications include:

  • Unexplained pleural effusion
  • Suspected pleural cancer or malignancy
  • Pleural thickening or abnormal pleural lesions
  • Recurrent pleural fluid
  • Selected pleural infections
  • Evaluation of other unexplained pleural diseases

The British Thoracic Society states that thoracoscopic or image-guided pleural biopsy may be used for diagnosing an undiagnosed unilateral pleural effusion, depending on the clinical indication and availability of the technique.

Thoracoscopy for Pleural Biopsy

One of the important uses of thoracoscopy is obtaining pleural tissue biopsies. During the procedure, the pulmonologist can identify abnormal areas and collect targeted samples.

Pleural biopsy may help diagnose conditions such as:

  • Pleural malignancy
  • Tuberculosis
  • Other infections
  • Inflammatory pleural diseases
  • Other causes of unexplained pleural thickening or effusion

The biopsy samples are examined by a pathologist to help establish the diagnosis and guide further treatment.

Thoracoscopy for Pleural Effusion

Thoracoscopy can also be used in selected patients with recurrent pleural effusion. During the same procedure, fluid may be drained and, when clinically appropriate, talc pleurodesis may be performed to help reduce recurrent fluid accumulation.

The most appropriate approach depends on the underlying cause, patient's overall health, lung expansion and treatment goals.

How Is Thoracoscopy Performed?

Medical thoracoscopy is generally performed with local anaesthesia and sedation, although the exact approach depends on the patient and procedure. The chest is carefully assessed, and the insertion site may be identified using imaging such as ultrasound.

A small opening is made in the chest wall, through which the thoracoscope is introduced. The doctor examines the pleural cavity, obtains biopsies and drains fluid when required. Patients are monitored throughout the procedure.

Benefits and Recovery

Thoracoscopy can provide direct visualization of the pleura and targeted tissue sampling through a minimally invasive approach. In appropriate patients, diagnosis and selected pleural treatment may be performed during the same procedure.

Patients may experience temporary chest discomfort, cough or soreness after the procedure. As with any invasive procedure, risks can include bleeding, infection, pain and other complications, which should be discussed with the treating specialist beforehand.
